Summary

Positives

  • Some patients say low-dose guanfacine noticeably calms brain fog, overstimulation, or hyperadrenergic symptoms and gives them more mental stamina. Reddit.

Negatives

Hurdles & Side Effects

  • Patients often start low, take it at night, and try to decide whether the benefit came from guanfacine alone or from pairing it with NAC. Phoenix Rising.

Caused new worse symptoms

Chronic Illnesses: ME/CFS
Symptoms Treated: Chronic Fatigue - Moderate improvement - 1-3 months Cognitive Dysfunction - Moderate improvement - 1-3 months
Side Effects Experienced: Dizziness - 1-3 months Lack Of Appetite - 1-3 months

I’m mild-moderate and have pretty good luck with most interventions I’ve tried. My hope for guanfacine was stabilizing my ups and downs some, increased cognitive capacity (cognitive exertion is my fastest symptom spiker), less fatigue. Three weeks in on 1mg extended release and I can’t tell if it’s helped those things because I have constant lightheadedness that makes basic functioning incredibly challenging. I already tend towards low blood pressure but have managed that with my electrolytes and CoQ10 in the past. That hasn’t helped with this sadly. In addition it’s destroyed my appetite (the thought of eating sounds disgusting, I feel ill 2/3 of the way through anything I eat), and destroyed my libido. Still can’t work much, still same ups and downs, still same fatigue, but also new bad symptoms. Probably going to quit.
